I have a 7-10 year-old Shimano poster of Lance in a TDF yellow skinsuit riding in a similar position. He has a huge lump in the middle of his back and I postulated that he and his aero advisors might have stuffed something 4" tall under his jersey to blend his head/helmet into his back. Either that or he has a huge barrel chest, and a very flexible mid-back. Now I see that it is the latter.

When I tried this position 3 years ago (for about 4 months), trying to ride in Rob English's Mango position, I ended up with super-sore glutes, hamstrings, and calves, and zero-load on the quads. I think it would take years of stretching and training to successfully develop this riding position.
